Providers – How to Refer

Who Can Refer?

Any provider including but not limited to doctors social workers, nurses, and teachers

How to Refer Patients:

  • PPO or uninsured: Call us directly at ext. 2

  • HMO (Cigna, Health Net, Blue Shield, GNP): Refer as you would refer to any other specialist

  • CalOptima: Fill out a COPTP form and fax to Cal Optima for authorization
